Registration Instructions:

Coventry Public Schools uses an online registration process.   Once you have completed the registration process and uploaded all required documents, one of our registrars will contact you to set up a phone interview.  If you are unable to upload the required documents, please bring the documents to the Coventry Public Schools Administration building, 1675 Flat River Rd. where we can assist you. You can also email the documents to Registration@coventryschools.net.

 

Please note that the registration system does not work from mobile devices such as cellular phones.

Required Documents for all Students PreK through Grade 12

Student must reside in Coventry with Parent/Guardian  

The following documentation must be provided at the time of registration.  These documents may be uploaded or emailed to registration@coventryschools.net             

  • Completed On-Line Registration

  • Birth Certificate,  Passport or Military ID  

  • Valid Driver's License or Photo Identification of Parent/Guardian Note: Expired licenses/ID will not be accepted

  • Court Documentation (Legal documentation proving custody or guardianship, if applicable)

  • Residency Evidence - A true copy of two (2) pieces of current residency evidence from the following:

 

Current Mortgage Statement or Purchase and Sales Agreement

Current Property Tax Bill

Copy of Fully Executed Lease/Rental Agreement

Current Electric or Gas Bill showing name and property address

Students grades 9-12 copy of last report card and or current high school transcript

 

  • Pediatrician/State Physical Form with Immunizations - All forms must be signed by your child’s doctor and must contain the most recent immunizations and up-to-date physical information (dated within one year).  Kindergarten-age children must show proof of  a lead screening and a vision screening and dental screening.

  • Any Additional Documentation  (For example,  please bring any information regarding Individual Education Plan,   504 Plans, Response to Intervention Services or English Language Learner Services) 

  • Child must be 5 years old on or before  September 1 to enter Kindergarten

It is recommended that you have your child screened every year at ages 3, 4 and 5 years old prior to Kindergarten (RIDE). Please fill out this form and our Child Outreach Coordinator will contact you to schedule a screening.
