Western Coventry Transportation Opt-In Survey

Good Afternoon,
Due to shrinking enrollment, Coventry Schools is looking at bus ridership and bus stop configuration to be more efficient. Western Coventry Elementary and Coventry High School are two schools that have buses that are continuously operating well below capacity. Therefor, we are piloting a bus transportation Opt-in for these two schools.
Opting into bus transportation means that a parent or guardian is choosing to have their child participate in the school bus transportation program. By filling out and submitting this opt-in form, the parent or guardian is requesting that their child be transported to and from school using the busing service provided by Coventry Public Schools.
PARENTS/GUARDIANS MUST COMPLETE THIS FORM IF THEY PLAN ON HAVING THEIR CHILD USE THE SCHOOL BUS.
This process is for students who qualify for bus transportation as determined by the Transportation Policy (link below). Some students are within walking distance to their school, which is determined based on the most direct, safe route from the school to the student’s dwelling (not crossing 102 or 117), and don't qualify for a school bus.
