Student Registration

The following are requirements for PreK and Kindergarten entry in RI per RIGeneralLaw 16-21-SCHO

1. Physical exam performed no longer than 12 months prior to entry

2. Immunizations (Dtap, IPV, MMR, Hib,Hep B, varicella, pneumococcal, Hep A, rotavirus, flu)

3. Lead screening or letter of exemption by parent

4. Vision screening performed by health care provider or optometrist

5. Dental screening performed by dentist/dental hygienist 

 

Also if your child has not had a Child Outreach Screening please fill out this form and our Child Outreach Coordinator will reach out to schedule one.

For All Other Students:

You will be completing an online registration process.   Once you complete the registration process one of our registrars will contact you to set up a phone interview.  If you are unable to upload the documents required to register your student, there will be a drop box at the Coventry Public Schools Administration building where you can drop off copies of the required documents or you can email them to Reggateway@coventryschools.net.

Please note that the registration system does not work from mobile device such as cellular phones.

  • If you already have an Aspen account with Coventry Public Schools - sign in using your current user id and password

 

 

 

 

Checklist for Registration:

Proof of Age:  one of the following:

Original birth certificate

Valid passport

 

Immunization status:

Physical Exam

Immunization Record

 

Proof of residency - at least one document from each category A and B:

Category A

Utility Bill- example: gas, electric or water bill (cable, internet and cell phone are not utility bills)

Mortgage Statement

Purchase & Sale Agreement

Lease Agreement

Government Housing Form.

Category B

Valid driver's license

Valid state photo identification

 

Other documents if applicable:

 

Individualized Education Program (IEP) Special Education Records

 

Court or Custody Papers
